- "District Court" Forms listed below are used when the child support is paid directly to the other parent.
- "Expedited Process" forms ("ex pro") listed below are used when the County Child Support Office is involved in the case. The County would be involved in the case either because one of the parties receives or formerly received public assistance (cash grant, medical assistance, and/or child care subsidy) or because a party has applied for child support enforcement services in a county child support office.
- 4th District (Hennepin County) Family Court: all motions (except "ex pro" child support) filed by a party without an attorney MUST be screened by the Hennepin Family Court Self-Help Center before filing.
