NEW! Reinstatement of Driver's License forms are now available in the Civil, Criminal and Motor Vehicle categories. NOTE: Under the law at MN Statute §171.19, these forms CANNOT be used in cases where a license was revoked OR suspended for 1) failure to take or pass an alcohol or controlled substance test; OR 2) failure to pay court-ordered child support or maintenance. You should get help from a lawyer for those types of cases.
