Family Court Forms at SHC

Family law cases usually involve a lot of documents and many steps. The Self-Help Center has "facilitators" (people who give one-on-one help) who can answer questions and review completed forms, but they do not give legal advice.

Before filling out your forms, you may want to get an overview of the family court process by watching one of the following videos:

Court forms that can be used in any Minnesota District Court can be downloaded for free from the MN Court Forms Catalog. The Hennepin Court Family Court also publishes some forms on their website at Hennepin Family Court Forms. You may also buy printed form packets at the Self-Help Center.

Step 1:  Search the MN Court Forms Catalog

Step 2:  Select the topic Category in the catalog that fits your legal issue

Each category has a list of available forms. Some forms are bundled into a [Packet] that includes step-by-step instructions and all of the forms required to handle that specific legal process.

Step 3:  Choose the Statewide or District form

Statewide forms can be used in all MN District Courts. Forms for specific Districts may only be filed in that District Court. These Districts publish certain forms on their webpages: 1st District, 2nd District and 4th District.

Step 4:  Download forms in Word or PDF format

Most of our PDF forms are fillable, but due to Adobe licensing restrictions, you can print your completed forms but you cannot save them after filling them out.

  • Summary Dissolution of Marriage $5.00 (available in limited cases with no children, no real estate, limited assets and debts, a short-term marriage, and no domestic abuse)
  • Answer and Counter-petition for Dissolution of Marriage without Children (to respond)
  • Request for Temporary Relief without Children  $5.00 (to ask for support or other help pending the final divorce decree)
  • Response to Request for Temporary Relief without Children   $5.00
