Customer Survey

Collecting qualitative data is important in measuring the impact our services have on the courts and the public. We are asking colleagues and partners to help administer a Survey to customers who use the Self-Help workstations and Call-Center service.

About the Survey

  • 8 questions
  • Double-sided, with English on one side and Spanish on the other.

Survey Process

  • Please provide survey forms at each workstation location along with a drop-off location for completed surveys (e.g., small box, envelope, basket at counters).
  • Telling pro se customers about the survey will likely produce higher response rates.
