PLEASE READ AND COMPLETE THE JURY SUMMONS AND QUESTIONNAIRE. PLEASE RETURN THE QUESTIONNAIRE IN THE ENCLOSED SELF-ADDRESSED STAMPED ENVELOPE WITHIN TEN DAYS. IF THE SUMMONED JUROR NO LONGER LIVES AT YOUR ADDRESS (student, moved, etc) PLEASE WRITE THAT INFORMATION AND THE NEW ADDRESS ON THE YELLOW FORM AND MAIL IN THE ENCLOSED ENVELOPE FOR THEM. IF YOU ARE NOT A RESIDENT OF CARLTON COUNTY, PLEASE ANSWER NO TO QUESTION 3 AND INDICATE YOUR COUNTY OF RESIDENCE. IF YOU HAVE A CHANGE OF ADDRESS, PLEASE PRINT ON THE BOTTOM OF THE FORM.

JURY TERM:Jury service is for a maximum of 5 reporting days within a two month period (orientation counts as 1 day); or if you are chosen as a juror, you will be excused upon completion of the trial; or approximately two months; whichever occurs first, unless you are excused by the Court. In some instances, you may be required to serve longer than the above guidelines because of an unexpected court calendar. You will be notified if this should happen. Unless otherwise instructed, Court is in session Monday through Friday, from 9:00 a.m. to 12:00 Noon and from 1:00 p.m. to 4:30 p.m.

LOCATION:The Carlton County Courthouse is located in downtown Carlton on Highway 61 near the intersection of Highway 210 in Carlton, Minnesota.

PARKING:You may park in the parking lots located on the north and south sides of the Courthouse. If the lot is full, street parking is available.
