IMPORTANT! Make sure you choose forms from the correct sub-category:
"Expedited Process" forms (#CSX...) are used when the County Child Support Office is involved in the case, like when one of the parties gets or used to get public assistance (cash grant, medical assistance, and/or child care subsidy), or because a party has applied for child support services in a County Child Support Office.
"District Court" Forms (#CSD...) are used when the child support is paid directly to the other parent and the County Child Support Office is NOT involved.
Hennepin County (4th District) Family Court: all motions filed by a party without an attorney MUST be screened by the Hennepin Family Court Self-Help Center before filing, EXCEPT "ex pro" child support.
